Care Manager MSW / LCSW (MSW or LCSW) - Providence Sacred Heart Medical Center & Children’s Hospital, Spokane, WA Job Types: Full Time, Part Time Shift: Day Job Summary The Inpatient Care Manager, both MSW and LCSW, provides comprehensive patient‑centric management services for at‑risk patients in an acute care setting. Responsibilities include the assessment and planning of care, coordination with interdisciplinary teams, discharge planning, documentation of interventions, regulatory compliance, and patient advocacy. The goal is to ensure continuity of care for vulnerable patients by identifying and addressing social, financial, cognitive/behavioral, and legal barriers to care access. Responsibilities Conduct comprehensive assessments and develop individualized care plans Coordinate interdisciplinary care and facilitate communication among providers Plan safe and timely discharges, including post‑discharge follow‑up and resource linkage Document interventions and maintain accurate case records Ensure compliance with regulations, policies, and quality metrics Advocate for patients to overcome social and financial barriers to care Care Manager MSW Eligibility Required Qualifications Master's Degree in Social Work from an accredited CSWE program Washington Clinical Independent Social Worker Associate License 1 year of Social Work experience in any healthcare setting 1 year of experience in an acute care setting or completion of a TIPs or Case Management Orientation Program Preferred Qualifications ACM/CCM Certification Basic understanding of acute medical–surgical healthcare terminology and prior case‑management experience Care Manager LCSW Eligibility Required Qualifications Master's Degree in Social Work from an accredited CSWE program Washington Clinical Independent Social Worker License 2 years of Social Work experience in any healthcare setting 1 year of experience in an acute care setting or completion of a TIPs or Case Management Orientation Program Preferred Qualifications Basic understanding of acute medical–surgical healthcare terminology and prior case‑management experience Salary Care Manager MSW (Eastern Washington – Richland, Spokane, Walla Walla): Min: $32.92 / hr, Max: $51.11 / hr Care Manager LCSW (Eastern Washington – Richland, Spokane, Walla Walla): Min: $37.04 / hr, Max: $57.51 / hr Benefits Providence offers a comprehensive benefits package, including a 401(k) retirement plan with employer matching, medical, dental, and vision coverage, life and disability insurance, paid parental leave, vacation, holidays, health‑related time off, and additional voluntary benefits.
