Compensation: $225,000 – $275,000 Base Salary + Annual Bonus Opportunity

The Role

You will provide comprehensive outpatient primary care, including preventive medicine, acute care, health maintenance, and chronic disease management (hypertension, diabetes, dyslipidemia, obesity).

Our integrated environment offers seamless care coordination alongside cardiologists and specialists for complex cardiovascular cases. You will also have the opportunity to provide clinical leadership to our care team and help shape the future of our practice.

Qualifications

• Degree: MD or DO from an accredited institution
• Certification: Board Certified or Board Eligible in Internal Medicine or Family Medicine
• Licensure: Active, unrestricted New York State medical license and DEA registration
• Experience: 2+ years of outpatient/private practice experience preferred
• Skills: Strong diagnostic, chronic disease management, and interpersonal communication skills
• Technology: Comfortable with EHR systems and modern digital health tools
• Focus: Interest in cardiovascular prevention and integrated patient care
