Key Responsibilities\n

• Design and implement individualized treatment and discharge plans that incorporate appropriate social work interventions, ensuring patients and families are actively involved in their care pathways.

• Collaborate closely with healthcare professionals, including physicians and nurses, to develop integrated care plans that address the holistic needs of patients.

• Facilitate effective discharge planning across multidisciplinary teams, liaising with third-party payers and other healthcare providers to ensure smooth transitions of care.

• Provide counseling and education to patients and families about adapting to health challenges, utilizing crisis intervention techniques as necessary to support acute situations.

• Engage in individual, group, and family therapies as needed, utilizing your expertise to foster therapeutic relationships and promote healing.

• Participate in on-call rotations to respond to after-hours needs, ensuring consistent support availability to patients.

• Offer educational presentations at conferences, sharing knowledge and expertise in psychosocial oncology practice with colleagues and community professionals.

• Engage in community service activities, promoting access to resources for cancer survivors and other patients through education, consultation, and volunteer efforts.

• Contribute to the professional development of the social work field by mentoring new social workers and participating in training initiatives.

Patient Population / Client Focus\n

You will work primarily with patients dealing with a range of psychosocial issues related to health challenges, including but not limited to oncology, chronic illnesses, mental health disorders, and familial stressors. Qualifications\n

• Master's degree in Social Work (MSW) from an accredited institution.

Compensation and Benefits\n

The salary for this position ranges from USD 70,000 to 100,000 per year, commensurate with experience and qualifications.
