I. GeneralPurposeoftheJob

Providesmedicalcareinaccordancewithbestcurrentstandardsofprofessionalpractice

II. EssentialDuties&Responsibilities

• 1. Takeshistories,performsphysicalexamination,prescribestreatmentandmedicationforconditions falling in the scope of his/her specialty following evidence based medicine.


• 2. Design and participate in qualityimprovement activities, includingMedical Record Peer Review, performance and procedure audits, outcome audits and focused reviews.


• 3. Will provide relevant information about the Patient Centered Medical Home (PCMH) and Chronic Care Management program (CCM) program including brochures, that describe the valuesandgoalsoftheprogramtopatients.Theinformationdisseminatedwillbebaseduponthe person’s role as it relates to patient care within LISH.


• 4. Will obtain a waiver if not already on file for Medically Assisted Treatment for those suffering substanceusedisorders.Thiswillincludeobtainingthewaiverwithin60daysofhire.Timewill be allotted during the hours of LISH to accomplish this.


• 5. Will becomeproficientintelemedicineandprovidecareviathisplatformasrequired.


• 6. Will assistintheeducationalgrowthoftheagencybyallowingstudentstoshadowas needed.


• 7. WillbeavailableforcommitteessuchasQualityassurance,peerreviewandrootcause analysis to provide periodic input into the work flow and improvements of the agency.


• 8. WillbeavailableforOn-Call/After-hoursassignmentsasneeded.


• 9. Coordinatestests,referralsandcareofLISHpatientsoutsidetheHealth Center.


• 10. Provides the patient or representative information necessary to give consent prior to start of any non-emergency procedure or treatment or both.


• 11. Ensurespromptfollow-upactiononpatientswithabnormaltestresultsorphysical findings.


• 12. Setsstandardsforpreventivecareanddiagnosticscreeningwithinhis/herspecialty;participatesin the development of programs to carry out these standards.


• 13. Providessafeandsuccessfulperformanceincludingmeetingproductivity,qualityandpatient satisfaction standards.

III. SupervisoryResponsibilities

ResponsibleforsupervisingavarietyofclinicalsupportstaffincludingLPN’s,RN’s,and/orNurse Practitioners as needed.

MedicalDoctoralDegree(MD/DO)

MustbelicensedbyandcurrentlyregisteredwiththeNewYorkStateDepartmentofEducation.

Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions,anddecimals.Abilitytocomputerate,ratio,andpercentandtodrawandinterpretbargraphs.

Abilitytoapplycommonsenseunderstandingtocarryoutinstructionsfurnishedinwritten,oral,or diagram form.Ability to deal with problems involving several concrete variables in standardized situations.

ComputerSkills

Toperformthisjobsuccessfully,anindividualshouldhavebasictointermediateknowledgeofMicrosoft Office software (Outlook,Excel,Word),as well as Practice Management and Electronic Medical Record softwares).
