Hospice Care Coordinator: Growth & Community Partnerships

Transitions Care

Transitions Care is looking for a Care Coordinator in Delaware, Ohio to foster relationships with healthcare providers and generate referrals for hospice services. Candidates should possess a bachelor’s degree along with 3+ years of sales experience, preferably in healthcare. Strong communication and organizational skills are essential, as well as the ability to manage a territory independently.
