Key Responsibilities

Evaluate and treat patients in a skilled nursing facility setting to restore function and improve mobility
Develop and implement individualized physical therapy treatment plans based on patient needs and goals
Document patient progress, treatment outcomes, and clinical notes in accordance with facility and regulatory standards
Collaborate with interdisciplinary care teams to coordinate comprehensive patient care
Educate patients and their families on therapeutic exercises and home care strategies

Required Qualifications

Active Physical Therapist (PT) license in the state of Missouri or ability to obtain prior to start
Graduate of an accredited Physical Therapy program
Experience working in a skilled nursing facility (SNF) or similar post-acute care setting
Strong clinical documentation and communication skills

Preferred Experience / Skills

Prior experience with geriatric or post-acute patient populations
Familiarity with MDS and Medicare documentation requirements in a SNF environment
