Job Title: Physical Therapist (PT) – Skilled Nursing Facility (SNF)

Location: Seattle, WA 98126
Job Type: Travel / Contract [NO BENEFITS]
Contract Duration: 13 Weeks
Shift: Day Shift
Schedule: 5×8-Hour Shifts, 8:00 AM – 4:30 PM
Experience Required: Minimum 1 Year

Pay Package

• Total Weekly Pay: $2,505

Required Certifications & Licensure

• BCLS (Basic Cardiac Life Support) certification.
• Valid state Physical Therapist (PT) license.
• Candidates with a pending PT license may be considered.

Required Qualifications & Skills

• Minimum 1 year of experience in a Skilled Nursing Facility (SNF) and/or Long-Term Care (LTC) setting.
• Previous experience treating adult and geriatric patients.
• Experience with PointClickCare charting is required.
• Strong clinical assessment, communication, and patient care skills.
• Ability to develop and implement individualized physical therapy treatment plans.

Job Responsibilities

• Provide physical therapy services to adult and geriatric patients in a skilled nursing and long-term care environment.
• Evaluate patients' mobility, muscle strength, balance, coordination, and functional abilities.
• Develop and implement individualized treatment plans to improve mobility, physical function, and independence.
• Monitor patient progress and adjust treatment plans based on clinical assessments and patient needs.
• Document evaluations, treatment sessions, patient progress, and outcomes using the facility's PointClickCare charting system.
• Collaborate with the interdisciplinary healthcare team to support patient recovery, safety, and overall well-being.
• Educate patients and caregivers on therapeutic exercises, mobility techniques, and strategies to support functional independence.
