With your expertise, you will assess patient needs, implement care plans, and collaborate with interdisciplinary teams. Key Responsibilities: Conduct initial and ongoing assessments of patient needs. Implement and adjust individualized care plans. Ensure compliance with hospice policies and Medicare regulations. Educate patients and families on self-care and end-of-life options. Supervise paraprofessionals delivering patient care. Document and report clinical findings and participate in interdisciplinary meetings. Enhance patient satisfaction and experience. Qualifications: Current Michigan LPN License in good standing. Minimum of 1 year experience in hospice or palliative care. Strong empathy and communication skills. Flexibility to respond to varying patient needs.
