Patient Navigator

Join our Community Health Access team as we assist neighboring communities in accessing healthcare resources. The Patient Navigator supports individuals and families by finding resources, aiding completion of health coverage or public programming, and participating in community outreach events. Identify potential clients, complete initial needs assessment, and maintain follow-up to ensure appropriate guidance to health services. Provide informal counseling, education, and social support to clients. Assist them with utilizing available resources, including scheduling medical appointments, accompanying them to appointments, and assisting with completion of medical program applications. Develop and maintain strong working relationships with external referral sources and community agencies and internal contacts (departmental team members). Maintain accurate client records by documenting all contacts, services provided, and outcomes to track all clients and services for future reference. Requirements Education: High school diploma equivalency with 2 years of cumulative experience or associate’s degree/Bachelor’s degree or 4 years of applicable cumulative job‑specific experience required.
