At Elara Caring, Physical Therapists provide high-quality care to patients in their homes, helping them regain strength and mobility while improving their quality of life. You will deliver skilled physical therapy services, conduct comprehensive assessments, develop and implement personalized care plans, and adjust therapeutic interventions based on patient progress.
Responsibilities:
- Provide direct patient care, evaluate functional needs and outcomes, and collaborate with other healthcare professionals.
- Design and implement individualized plans of care in line with patient goals and family participation.
- Document evaluations, treatment goals, interventions, and patient progress in accordance with clinical standards.
- Offer direction and support to therapy assistants and other staff members.
- Promote Elara Carings mission and ensure quality and compliance in care delivery.
Qualifications:
- Graduate of an accredited physical therapy program (APTA approved).
- Current, unrestricted PT license in the state of employment.
- Minimum one year of experience in a healthcare setting.
- Reliable transportation, valid drivers license, and auto insurance.
- Able to lift and transfer up to 50 pounds and travel within the assigned coverage area (up to 50%).
