We are seeking an experienced Pediatric Palliative Care Specialist, RN to join our nationally respected Transitions Kids program. What You'll Do

As a Pediatric Palliative Care Specialist, you will serve as a trusted clinical resource and advocate for children and families across the continuum of care.

Deliver Exceptional Clinical Care

• Assess, plan, coordinate, and provide comprehensive pediatric palliative care services
• Develop individualized care plans that reflect the unique goals, values, and needs of each child and family
• Provide symptom management, clinical support, and compassionate guidance throughout the care journey
• Deliver care in homes, healthcare facilities, and through telehealth as appropriate
• Provide after-hours support and visits as part of scheduled coverage responsibilities

Partner with Families

• Build meaningful and trusting relationships with children, parents, caregivers, and loved ones
• Support families during periods of uncertainty, crisis, and loss
• Facilitate goals-of-care conversations and help families navigate complex healthcare decisions
• Educate families regarding disease progression, symptom management, available resources, and care options

Collaborate Across Disciplines

• Partner with physicians, social workers, chaplains, child life specialists, therapists, and other interdisciplinary team members
• Coordinate services and community resources to maximize quality of life and family support
• Supervise and delegate care activities for LPNs and Hospice Aides as appropriate
• Participate in interdisciplinary team meetings and care planning discussions

Serve as a Pediatric Care Expert

• Maintain current knowledge of pediatric palliative and hospice best practices
• Support the orientation and development of new team members and students
• Ensure compliance with regulatory, accreditation, and organizational standards
• Contribute to continuous quality improvement and excellence in patient care

Required Qualifications

• Bachelor of Science in Nursing (BSN)
• Current Registered Nurse license in North Carolina
• Minimum of two years of recent pediatric clinical experience
• Current CPR certification
• Valid North Carolina driver's license
• Reliable transportation and automobile insurance
• Strong assessment, documentation, and clinical decision-making skills
• Ability to travel throughout the service area and provide care in patient homes and facilities
• Ability to lift and/or carry up to 50 pounds
• Strong technology and computer proficiency

Preferred Qualifications

• Pediatric hospice experience
• Pediatric palliative care experience
• Pediatric inpatient hospital experience
• Pediatric home health experience
• Pediatric Hospice and Palliative Care Nurse Certification (CHPPN)
• Bilingual skills, particularly Spanish
• Experience facilitating goals-of-care conversations and advance care planning discussions
• Knowledge of Medicaid regulations, accreditation standards, and reimbursement processes
