Responsibilities

• Evaluate and treat patients in a subacute/skilled nursing environment
• Develop and implement individualized treatment plans
• Focus on mobility, strength, balance, fall prevention, and safe discharge planning
• Collaborate with occupational therapy, speech therapy, nursing, and physicians
• Maintain accurate, timely, and compliant documentation
• Participate in care planning and interdisciplinary team meetings as needed

Qualifications

• Active Physical Therapist license in Nebraska (or eligibility for licensure)
• Graduate of an accredited Physical Therapy program
• Skilled nursing or subacute experience preferred; new graduates welcome
• Strong clinical reasoning and communication skills
• Patient-centered and team-oriented approach
