Physical Therapist

Schedule: 8 hours per shift

Assignment Details: Guaranteed Hours: 40 Contract Weeks: 16

Requirements:

• Current State License as a Physical Therapist - Required
• Proof of current CPR certification - Required
• Current driver's license. Must have reliable transportation and insurance - Required
• 1-2 years experience as a Physical Therapist in an acute care or rehabilitation setting - Required
• Previous home care experience - Preferred
• Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program - Required

Responsibilities:

• Conduct clinical assessments to determine patient needs and appropriateness of care.
• Document and prepare Plans of Care (POC), progress notes, and other clinical records.
• Provide treatment for neuromuscular, neurological, and orthopedic disorders.
• Instruct patients and families on transfer techniques and the use of therapeutic equipment.
• Offer education to patients and families regarding therapeutic procedures based on individual needs.
• Collaborate with interdisciplinary teams to ensure coordinated care.
• Adhere to infection control and universal precautions in patient care activities.
• Accept clinical assignments consistent with education and competency.
• Perform additional duties as assigned or requested.
