Responsibilities
- Provide emergency nursing care during home visits and urgent events within a hospice patient population.
- Conduct rapid assessments, monitor vitals, and respond to acute changes in patient condition; initiate appropriate interventions.
- Administer medications and treatments per physician orders; monitor for adverse reactions and efficacy.
- Collaborate with an interdisciplinary team (physicians, caregivers, hospice case managers, social workers) to coordinate patient care plans.
- Educate patients and families on symptom management, safety, and end-of-life care considerations.
- Document care in the electronic health record; ensure compliance with infection control and safety protocols.
- Prepare for and participate in transfers to higher levels of care when indicated; communicate handoffs clearly.

Requirements
- Active California Registered Nurse license
- BLS certification required (HS-BLS or RQI BLS acceptable)
- ACLS certification required
- HLFS (Hospital Fire and Life Safety) certification required
- Education: RN degree (Associate Degree in Nursing or higher)
- Experience: Emergency nursing experience preferred; ability to work effectively in a home healthcare/hospice setting
- Eligibility to work in the United States; ability to travel within the service area as assigned
