Seven Healthcare are seeking a dedicated Physical Therapist for a Travel Contract role in Owensboro, KY. This position is an excellent opportunity for a skilled therapist with a passion for home-based care and Medicare Oasis experience to join a supportive and dynamic work environment.

- Evaluate and treat patients based upon physician orders/referrals
- Complete comprehensive evaluations using appropriate tools and procedures
- Review patient records for current diagnosis, past medical history, precautions, and contraindications
- Establish measurable goals and develop treatment plans for maximum independence/function
- Regularly re-assess effectiveness of treatment plans and modify as needed
- Communicate patient needs and progress to the treatment team and family members
- Consult with medical providers regarding evaluation, treatment, progress, and discharge
- Recommend and assist in securing special adaptive equipment for patients
- Maintain and submit evaluation, treatment, and billing documentation as required
- Adhere to standards of professional practice and applicable laws
- Supervise Physical Therapist Assistants and Aides in patient care activities
- Attend internal staff development programs and maintain clinical practice skills
- KY PT License, Compact License Accepted
- Experience with Home Care Home Based and Medicare Oasis
