Admissions Coordinator

The Admissions Coordinator is responsible for managing the admissions process, including transfers and discharges, as well as fall reporting within the facility. This role requires strong organizational skills and attention to detail to ensure residents receive quality care and support during their transition into care.

Responsibilities

• Oversee and coordinate the admissions process, ensuring compliance with policies and procedures.
• Manage resident transfers and discharges effectively and in accordance with established protocols.
• Monitor and report any falls or incidents involving residents.
• Maintain accurate and up-to-date records related to admissions and discharges.
• Collaborate with nursing staff and caregivers to facilitate a smooth transition for residents.

Caregiver Ratios

The facility maintains specific caregiver ratios to ensure adequate resident support:

• Night Shift: 1 caregiver for up to 35 residents, with medication pass at the beginning of the shift and up to 2 Certified Nursing Assistants (CNAs) available.
• Day Shift: 1 caregiver for up to 35 residents, supported by a medication aide, with the nurse responsible for all treatments and up to 3 CNAs available, plus potentially 1 Bath Aide (Monday through Friday).

Additional Information

Clinicians are guaranteed clinical hours per week based on their scheduled shifts. Continuous operation runs from Sunday through Saturday, maintaining a structured work week.

It is crucial for all staff to adhere to established policies, as any sudden changes in staffing without appropriate notification may incur a processing fee. Housing may be provided and will follow local rate guidelines.
