Position requires a commitment of 40 hours per week. Scheduling may include 4x10's or 5x8's depending on operational needs. Candidates should have at least 2 years of case management experience. Experience must include 6 months of discharge planning in an acute care setting. Ideal candidates will have worked in a large organization or teaching hospital with 500 beds or more.
