Community Senior Care Coordinator/Nurse PractitionerAs a Community Senior Care Coordinator/Nurse Practitioner, you will be part of the largest multi-specialty care system in the Twin Cities. This position provides the primary health care for patients in assigned care settings in the Minneapolis/St. Paul metro area. This position provides care coordination to achieve patient centered, high quality and cost effective care across the continuum.QualificationsThe position requires certification as a Nurse Practitioner (Adult/Geriatric/Family) by a national certifying body (ANCC, AANP, ect.)A current unrestricted Minnesota APRN license as a Nurse PractitionerMust be able to provide own transportation for local travel. May provide service to patients at multiple clinic locations and/or hospital sites.Preferred QualificationsMinimum of one-year experience working in acute care preferred for those assigned to the transitional care settings.Knowledge of Medicare Guidelines preferred for those assigned to long-term/transitional care settings.3-5 years' experience working with seniors in a clinical settingYou will be joining a team that is supportive and respectful of one another and deeply committed to the mission of HealthPartners.
