Job Title Physical Therapy Assistant (PTA) Location Alamosa, Colorado Schedule / Job Type Contract | 13-Week Assignment | Days | Onsite Key Responsibilities Deliver physical therapy treatments to patients in a skilled nursing facility (SNF) setting under the supervision of a licensed Physical Therapist Assist in implementing individualized treatment plans to support patient rehabilitation and functional goals Monitor and document patient progress, reporting changes in condition to the supervising Physical Therapist Educate patients and caregivers on therapeutic exercises and home programs as directed Maintain accurate and timely clinical documentation in accordance with facility and regulatory standards Required Qualifications Active Physical Therapy Assistant (PTA) license in the state of Colorado or eligibility to obtain Graduate of an accredited Physical Therapist Assistant program Current CPR/BLS certification Experience working in a skilled nursing facility (SNF) or similar clinical setting Preferred Experience / Skills Prior travel or contract therapy experience Familiarity with SNF documentation and Medicare compliance requirements Why Join Us Opportunity to gain valuable experience in a skilled nursing environment through a structured 13-week contract assignment Supportive placement process with a staffing team dedicated to matching clinicians with the right opportunities Concentric
