Physical Therapist (PT) / Physical Therapist Assistant (PTA) - PRN

We are seeking PRN Physical Therapists (PT) and Physical Therapist Assistants (PTA) to support our skilled nursing therapy team on an as‑needed basis. These roles are ideal for licensed clinicians looking for flexible scheduling while delivering high‑quality rehabilitation services in a skilled nursing environment. Key Responsibilities PT Responsibilities: Perform evaluations and develop individualized physical therapy treatment plans Supervise PTA treatment when applicable Collaborate with interdisciplinary teams to support resident goals and discharge planning PTA Responsibilities: Provide physical therapy treatments under the direction of a licensed PT Carry out treatment plans to improve mobility, strength, balance, and functional independence Communicate resident progress and concerns to the supervising PT For Both Roles: Provide skilled therapy services to short‑term rehab and long‑term care residents Complete accurate and timely documentation in accordance with Medicare and facility guidelines Participate in care planning and interdisciplinary communication as needed Qualifications Active PT or PTA license in AZ.
