RN Case Manager - Compact State Licensed

Managing treatment plans for patients, the full-time RN Case Manager - Compact State Licensed will assess health needs, coordinate care, and monitor progress remotely while collaborating with healthcare providers and community resources. Key responsibilities Assessing members' health status and coordinating care, including inpatient reviews and discharge planning Developing and implementing treatment plans while identifying alternative care options in line with cost containment guidelines Monitoring health status and treatment effectiveness, adjusting care plans as necessary based on client needs Required qualifications 5+ years of related work experience in clinical nursing and patient assessment Bachelor's degree or equivalent work experience in a related field Graduate of an accredited school of nursing RN - Registered Nurse - State Licensure with Compact State Licensure in good standing Knowledge of medical terminology and cost containment practices in healthcare
