Advocate Health is seeking a Registered Nurse (RN) Case Manager for Home Health in Illinois. The role is full-time with 40 hours/week, Monday–Friday, 8:00 AM–4:30 PM, plus weekend and on-call rotations. A $3,000 sign-on bonus is available for external candidates. Responsibilities include developing therapeutic nurse-patient relationships, ensuring quality care, and patient education.
