Physical Therapist - Travel Contract

Minimum Requirements
Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
Active Kentucky Physical Therapy license required to start the assignment.
BLS certification from AHA or American Red Cross may be required.
Must have at least 12 months of Home Health physical therapy experience within the past 3 years. (Client requirement.)
Experience with HCHB (Homecare Homebase) EMR preferred.
Ability to work in a hospital-based home health territory and follow agency policies.

Assignment Details
Facility Type: Hospital (Home Health service line).
Shift Name: Days.
Shift Details: Monday–Friday, 8:00 AM–5:00 PM, 8-hour days.
On call: One weekend per month on call.
Length of Contract: 13 weeks.
Positions Available: 1.
