- Conduct thorough evaluations for patients in both inpatient and outpatient environments, creating personalized treatment plans
- Deliver physical therapy interventions focusing on enhancing mobility, strength, balance, and functionality
- Work collaboratively with nurses, physicians, case managers, and other rehabilitation professionals to optimize patient outcomes
- Instruct patients and their families on therapeutic goals, discharge strategies, and at-home exercises
- Document assessments, treatments, and patient progress in line with facility and regulatory standards
- Engage in care team meetings and interdisciplinary discussions to ensure seamless patient care
- Doctorate in Physical Therapy (DPT) or an equivalent qualification from an accredited program
- State licensure as a Physical Therapist
- CPR certification preferred
- Specialized certifications (e.g., neurological, geriatric) are advantageous but not mandatory
- Prior experience in physical therapy is highly desired
- Ability to provide care to patients with complex medical and mobility challenges
- Excellent communication and teamwork abilities
- Familiarity with digital charting and clinical documentation software
