Continuum Therapy Partners is seeking a compassionate and dedicated Physical Therapist (PT) for a unique full-time split position serving Evergreen Nursing & Rehab in Effingham, Illinois, and Helia Healthcare of Olney in Olney, Illinois .

This opportunity allows you to provide high-quality physical therapy services across two supportive skilled nursing facilities while working alongside collaborative teams dedicated to resident-centered care and positive patient outcomes.

As a Physical Therapist (PT) , you will help adult and geriatric patients improve mobility, strength, balance, endurance, and functional independence while delivering individualized rehabilitation services.

Responsibilities

• Evaluate and treat patients in a skilled nursing facility setting.
• Develop and implement individualized physical therapy treatment plans.
• Provide skilled physical therapy interventions to improve mobility, strength, balance, and independence.
• Monitor patient progress and adjust treatment plans based on patient needs.
• Collaborate with physicians, nursing staff, caregivers, and interdisciplinary rehabilitation team members.
• Complete accurate and timely documentation according to company, state, and federal guidelines.
• Educate patients and families on mobility strategies, therapeutic exercises, and safety techniques.
• Provide compassionate, evidence-based physical therapy services focused on improving quality of life.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Illinois Physical Therapist (PT) license required.
• Skilled nursing or rehabilitation experience preferred but not required.
• New graduates are encouraged to apply.
• Strong communication, organization, and teamwork skills.
• Ability to provide coverage across assigned facilities.
