We are seeking a compassionate, highly organized, and dedicated Care Coordinator to join our team. This role serves as the primary point of contact for clients and caregivers, helping coordinate services, develop individualized care plans, and ensuring clients receive the support they need. Exceptional written and verbal communications skills are a must as well as having the ability to interact effectively with professionals in a variety of settings including medical settings and the community. Must be passionate about helping seniors live with dignity. The ideal candidate will have a minimum of one year of experience in healthcare, social work, or a related field. They should thrive in a fast-paced environment and demonstrate exceptional multitasking abilities while maintaining a high level of accuracy and attention to detail. Previous scheduling experience is preferred but not required. Success in this role requires outstanding planning and organizational skills, strong critical thinking and problem-solving abilities, and the ability to effectively prioritize competing responsibilities. Candidates must also be proficient in the Microsoft Office Suite and comfortable using computers and electronic systems in a professional healthcare setting.

Compensation: $40,000.00 - $60,000.00 per year
