Soliant is seeking experienced Patient Care Coordinators (RN Case Managers) to support acute inpatient populations across MD/VA facilities. This role focuses on care coordination, discharge planning, and ensuring safe, cost-effective patient transitions. Responsibilities
- Complete initial face-to-face patient assessments within 24 hours of admission
- Coordinate and document discharge planning and care transitions in EMR systems
- Collaborate with physicians, nursing, utilization review, patients, and families
- Arrange post-acute services including home health, DME, SNF, rehab, and infusion needs
- Active Associate Degree in Nursing (ADN) required
- Minimum 1 year acute care experience (Med/Surg, ED, or ICU)
- Experience with EMR systems required
- Knowledge of Medicare Advantage, Medicaid, commercial payers, and CMS regulations
- Completion of PCC Assessment of Critical Skills required
- Case management experience preferred
- Every other weekend required
- Credentialing: Expedited process approved
- Strong collaboration with hospital-based physicians and care teams
