Job Summary:
Physical Therapist (PT) specializing in Home Health in Hilo, Hawaii. This position involves providing rehabilitation services within a Skilled Nursing Facility (SNF) and/or Intermediate Care Facility (ICF). The role includes performing evaluations, treatments, and developing individualized care plans to maximize patient independence and quality of life. The candidate must hold a current Hawaii license and be available to start on 09/14/2026.

Job Details:

• Location: Hilo, Hawaii
• Schedule: Rotating shifts, generally scheduled five (5) days per week, including one weekend day. Shift hours are 8 hours per day, with a minimum of 40 hours per week.
• Required Qualifications: Current Basic Life Support (BLS) certification; Current State of Hawaii license in good standing appropriate to the discipline; Minimum two (2) years of experience in the SNF/ICF setting preferred.
• Preferred Qualifications: No additional qualifications specified.
• Float: Float rotation may be required based on census and staffing needs.
• Additional Information: Must arrive the Thursday before the start date to attend employee health appointment on Friday. Employees are expected to perform duties within their scope of practice, including evaluations, treatments, documentation, and supervision of therapy assistants when applicable. The role may involve floating to other units or facilities based on operational needs.
