Licensed Practical Nurse Home Health

Job Description We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our homecare team. In this role, you will perform a variety of delegated nursing tasks and provide essential patient care within your Scope of Practice, under the guidance of registered nurses, licensed physicians, and other healthcare professionals. As an LPN in home care, you will deliver compassionate, skilled care that allows patients to remain in the comfort of their own homes. Responsibilities Supports the admission and check‑in process, assisting patients and families with understanding routines, rights, and the care setting Helps complete necessary forms and documentation Collects and records essential assessment data, including vital signs, height and weight, medication history, and billing details Identifies and responds appropriately to patient pain or discomfort Assists patients with Activities of Daily Living (ADLs) to promote comfort and independence Prepares patients for medical or surgical interventions Performs interventions within the LPN Scope of Practice Supports physicians and licensed independent practitioners with treatments and procedures Obtains and tracks laboratory specimens (e.g., venipunctures, stool, urine) for analysis Prepares, administers, and manages medications (oral, IM, SQ, rectal, intradermal) in accordance with protocols Helps maintain supplies and ensures proper stocking and organization Minimum Qualifications Current LPN License in the state of practice Basic Life Support (BLS) Certification for healthcare providers LPNs hired into hospital departments (excluding long‑term care or behavioral health settings) must enroll and complete their RN within four years Approved IV Certification, as required by facility Clean driving record, as required by facility Ability to connect to the company’s online database twice daily, as required by facility Preferred Qualifications One (1) year experience in a clinic or acute hospital nursing setting One (1) year specialty clinic experience Strong visual and auditory perception for interpreting patient needs, alarms, and equipment Manual dexterity for manipulating complex equipment and frequent computer use Ability to lift, transfer, and assist patients, including use of gurneys and other equipment Ability to walk, stand, and work in busy patient rooms Benefits Comprehensive benefits package covering health, wellness, and financial security Intermountain Health PEAK program – up‑front tuition coverage and learning options for caregivers Schedule & Compensation Shift: 9:00AM–5:00PM, full time (40 hours per week) Hourly rate: $26.74 – $41.28, depends on experience Location Las Vegas, Nevada (Intermountain Health Home Services – Home Health at Silverado Ranch) Equal Opportunity Employment Statement Intermountain Health is an equal‑opportunity employer.
