PRN Physical Therapist — South / Southwest Chicago Suburbs About the Job Versed is partnering with Loyal Home Health to hire a PRN Physical Therapist to support patients across the South and Southwest Chicago suburbs, primarily in Joliet, Plainfield, Orland Park, Lockport, and Lemont . This opportunity is ideal for a Physical Therapist who values flexibility and autonomy and wants to add a consistent home health caseload within a defined local territory. It may be especially attractive to a PT already working in home health who is looking to add additional visits around an existing schedule. The company currently has approximately 10–12 visits per week available in this territory, with the opportunity for the right PT to absorb most or all of that volume depending on availability. The patient population is primarily orthopedic and post-acute, with some patients requiring multiple visits per week. Depending on the case, the PT may also complete Start of Care visits and admissions, including PT-only cases. You’ll have the autonomy to manage your own patient schedule and route while working closely with a collaborative clinical and therapy team. Loyal Home Health is an independently operated home health agency that has continued to grow while maintaining a hands‑on, clinician‑focused culture. Leadership remains closely involved in operations, and directors and managers are willing to step in to support clinicians when coverage is needed. This is an excellent opportunity for a PT looking for flexible PRN work, meaningful additional patient volume, geographic consistency, and the ability to build visits around other professional commitments. Territory Primary coverage includes: Joliet, IL Plainfield, IL Orland Park, IL Lockport, IL Lemont, IL Additional patient coverage may be available throughout surrounding South and Southwest suburbs, including Homer Glen, New Lenox, Mokena, Frankfort, Tinley Park, Romeoville, Bolingbrook, Shorewood, and nearby communities. The goal is to keep the caseload geographically aligned with the PT’s existing location and schedule whenever possible. What You’ll Do Provide physical therapy services to home health patients within the South / Southwest territory Manage approximately 10–12 visits per week , based on patient volume and your availability Perform evaluations, reassessments, treatments, and other skilled PT visits Complete Start of Care visits and admissions as needed, including PT-only cases Develop individualized plans of care focused on restoring mobility, strength, safety, and functional independence Treat primarily orthopedic and post-acute patients in the home setting Complete OASIS and other required clinical documentation accurately and on time Maintain timely documentation within HomeCare HomeBase (HCHB) Manage your own daily patient schedule and route planning Coordinate care with nursing, therapy, physicians, and other members of the interdisciplinary team Educate patients and caregivers on mobility, safety, treatment plans, home exercise programs, and recovery goals Maintain clear communication with patients, families, physicians, and the Loyal clinical team Who You Are Licensed Physical Therapist in Illinois Prior home health experience strongly preferred Comfortable working independently in a field-based home health environment Experience with orthopedic and post-acute patient populations preferred Experience completing Start of Care visits and admissions preferred Experience with OASIS documentation preferred Experience with HomeCare HomeBase (HCHB) strongly preferred Comfortable managing your own schedule and coordinating patient visits efficiently Strong clinical judgment and ability to work autonomously in patients’ homes Highly organized with strong time management and documentation skills Strong communication and relationship-building skills with patients, caregivers, and interdisciplinary teams Collaborative, dependable, and responsive Interested in flexible PRN work and adding additional home health visits to your existing schedule Why This Opportunity Approximately 10–12 visits per week currently available Defined South / Southwest suburban territory Ability to add visits around another home health or clinical position Primarily orthopedic and post-acute caseload Autonomy over daily scheduling and route planning Collaborative clinical and therapy team Hands‑on leadership and operational support Stable, independently operated home health organization #J-18808-Ljbffr
