Summary The Harry S. Truman Memorial Veterans' Hospital is seeking a highly motivated and clinically skilled Inpatient Clinical Pharmacist to join our Pharmacy Service. The pharmacist will provide comprehensive, evidence-based medication management for Veterans across multiple inpatient care areas. Learn more about this agency Duties Help VA Careers - Pharmacy : Duties include, but are not limited to: The Inpatient Clinical Pharmacist provides direct pharmaceutical care for patients in inpatient wards, CLC, ER, OR, GI Lab, Cardiac Cath Lab, PACT Clinics, Specialty Clinics, and CBOCs. Responsibilities include:- Reviewing and verifying medication orders for accuracy, safety, and clinical appropriateness.- Applying evidence-based practice to support optimal therapeutic decisions. - Preparing and dispensing sterile and non-sterile compounds, hazardous medications, and controlled substances in accordance with USP , USP , and VA policies. - Monitoring and adjusting drug therapy for anticoagulation, vancomycin, aminoglycosides, and medications requiring specialized dosing. - Ensuring adherence to VA formulary and P&T Committee guidance, including therapeutic interchanges. - Providing medication information to providers, nurses, and patients in a timely and accurate manner. - Dispensing outpatient prescriptions during after-hours coverage. Physical aspects associated with work required of this assignment are typical for the occupation and would generally not require a pre-placement examination. Work Schedule : Evenings, 7 days on, 7 days off. Friday-Thursday Tour - Friday-Sunday: 8:00 a.m.-8:30 p.m. - Monday-Thursday: 10:00 a.m.-9:30 p.m. - Holiday coverage (8:00 a.m.-8:30 p.m.) is provided by the pharmacist on duty that week. - Education-Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. Today the sole degree is Doctor of Pharmacy.)
- Graduates of foreign pharmacy degree programs meet the educational requirement if the 2 graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT).
- Licensure. Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16.
- Knowledge of professional pharmacy practice.
- Ability to communicate orally and in writing to both patients and health care staff.
- Knowledge of laws, regulations, and accreditation standards related to the distribution and control of scheduled and non-scheduled drugs and pharmacy security.
- Skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.
