Nurse Practitioner Chronic Care, Hybrid / In-Home Care Richmond, KY / Lexington KY This Is Not a Clinic Job. And That s the Point. If you became an NP to practice autonomously , build real patient relationships , and actually improve outcomes this role was designed for you.
We re looking for a Nurse Practitioner who wants to step away from rushed visits, crowded schedules, and productivity pressure, and instead deliver thoughtful, evidence-based care to complex patients where it matters most: in their homes .

Compensation & Benefits:
$120,000 $140,000 base salary (depending on experience) + generous Sign On bonus!
