What You'll Do
As a Case Manager, you'll become the primary advocate and coordinator for a caseload of individuals receiving services through RCS Empowers.
You'll work alongside participants, families, guardians, residential providers, and community partners to ensure every individual receives high-quality, person-centered support.
Responsibilities include:
- Manage an assigned caseload while developing meaningful relationships with participants.
- Develop, coordinate, and monitor Individual Program Plans (IPPs).
- Advocate for participant rights, choices, and personal goals.
- Coordinate services with families, guardians, residential providers, funding agencies, and community resources.
- Monitor participant progress and adjust supports as needs change.
- Provide guidance, encouragement, and problem-solving support.
- Maintain accurate electronic documentation and case records.
- Make referrals to community resources and additional RCS services.
- Collaborate with supervisors and interdisciplinary teams to ensure exceptional participant outcomes.
- Participate in continuous improvement efforts and agency initiatives.
- Help create an environment where every participant is treated with dignity, respect, and compassion.

What We're Looking For
We're searching for someone who is compassionate, organized, and motivated to help others succeed.
Qualified candidates will have:
- Bachelor's Degree (or equivalent experience) in Rehabilitation, Human Services, Social Work, Psychology, or a related field
- Experience working with individuals with disabilities preferred
- Experience supporting medically fragile individuals is a plus
- Excellent communication and relationship-building skills
- Strong organizational and time-management abilities
- Ability to work independently while collaborating with a multidisciplinary team
- Valid Wisconsin driver's license
- Good driving record
- Reliable transportation and insurance meeting agency requirements

Physical Requirements
This position requires the ability to safely perform essential job functions, including:
- Walking, standing, bending, kneeling, reaching, and lifting throughout the workday
- Assisting with participant transfers when necessary
- Working indoors and outdoors in varying weather conditions
- Operating a motor vehicle
- Using computers and electronic documentation systems
- Responding appropriately during emergency situations
