Licensed Practical Nurse (LPN)
The Licensed Practical Nurse (LPN) is responsible for overseeing and coordinating the healthcare needs of individuals supported by the organization. This position provides direct nursing care, administers medications and treatments, monitors health status, and ensures accurate clinical documentation. The LPN collaborates with physicians, registered nurses, therapists, families, and Direct Support Professionals (DSPs) to develop and implement individualized care plans that promote optimal health outcomes. Responsibilities include staff training, health education, medical appointment coordination, hospital follow-up, emergency response, and advocacy to ensure continuity of care. The LPN promotes a safe, respectful, and person-centered environment while maintaining compliance with regulatory requirements and nursing standards of practice.

Responsibilities
- Administer/Oversee administration of medications and treatments as prescribed by healthcare providers
- Monitor and ensure documentation of persons supported health status, including vital signs, symptoms, and behavioral changes
- Provide direct nursing care and support, and along with trainings to Direct Support Staff (DSP) for individuals with chronic health conditions, mobility challenges, and communication barriers
- Collaborate with RNs, physicians, therapists, and support staff to develop and implement individualized care plans
- Educate persons' supported, families, and staff on health-related topics, including hygiene, nutrition, diabetes, and medication management.
- Accompany individuals to off-site medical appointments, hospital visits, and rehabilitation facilities as needed
- Visit and monitor individuals while hospitalized to ensure continuity of care and advocate for their needs
- Respond to medical emergencies and follow established protocols
- Maintain accurate and timely documentation in compliance with regulatory standards
- Promote a safe, respectful, and inclusive environment for all individuals
COMPETENCIES
- Medication administration and clinical treatment management - Safely administers medications and treatments as prescribed, ensuring accuracy, adherence to physician orders, and compliance with regulatory and organizational standards.
- Health monitoring and assessment - Monitors individuals' health status through observation, vital sign measurement, symptom tracking, and identification of physical, emotional, and behavioral changes requiring intervention.
- Clinical documentation and recordkeeping - Maintains accurate, timely, and comprehensive medical documentation, ensuring compliance with state, federal, and agency requirements.
- Direct nursing care and support - Provides compassionate, person-centered nursing care to individuals with chronic health conditions, mobility limitations, and complex support needs.
- Staff training and clinical coaching - Educates, mentors, and supports Direct Support Professionals (DSPs) on healthcare protocols, medication administration, infection control, and individualized health needs.
- Individualized care planning and implementation - Collaborates with interdisciplinary teams to develop, implement, and evaluate person-centered healthcare plans that address medical, behavioral, and support needs.
- Interdisciplinary collaboration - Works effectively with RNs, physicians, therapists, case managers, and support staff to ensure coordinated care and positive health outcomes.
- Health education and wellness promotion - Provides education to individuals, families, and staff on health-related topics, including medication management, nutrition, hygiene, diabetes care, and preventive health practices.
- Medical appointment coordination and advocacy - Coordinates and accompanies individuals to medical appointments, facilitating communication with healthcare providers and ensuring appropriate follow-up care.
- Hospital liaison and continuity of care - Monitors individuals during hospitalizations, advocates for their needs, communicates with treatment teams, and ensures continuity of care throughout transitions.
- Emergency response and crisis management - Responds promptly and effectively to medical emergencies, applying clinical judgment and established protocols to protect the health and safety of individuals served.
- Person-centered care and advocacy - Promotes dignity, independence, choice, and self-determination while advocating for individuals' healthcare needs and rights.
- Regulatory compliance and quality assurance - Ensures nursing practices, medication administration, documentation, and health services comply with applicable regulations, agency policies, and best practices.
- Safety and risk management - Identifies potential health and safety concerns, implements preventive measures, and promotes a safe environment for individuals, staff, and visitors.
- Communication and interpersonal effectiveness - Demonstrates strong verbal and written communication skills when interacting with individuals supported, families, healthcare professionals, and staff.
- Observation, critical thinking, and clinical judgment - Assesses situations accurately, identifies potential concerns, and makes informed decisions to support effective healthcare delivery.

PHYSICAL REQUIREMENTS
Prolonged periods of sitting at a desk and working on a computer. While performing the duties of this job, the employee is regularly required to talk and hear. The employee frequently is required to sit; stand; walk; use hands to finger, handle or feel; and reach with hands and arms. The employee must frequently lift and/or move up to 10 pounds and occasionally lift and/or move up to 50 pounds.
Qualifications
EDUCATION AND EXPERIENCE REQUIREMENTS
- Current and valid LPN license in the state of New Jersey
- CPR and First Aid certification (or willingness to obtain upon hire)
- Experience working with individuals with IDD preferred, but not required
- Strong communication, organizational, and interpersonal skills
- Ability to work independently as a part of a multidisciplinary team
