About the Role: As an LPN at The Avenue at Strongsville, you will be responsible for providing exceptional nursing care to our residents and patients. This includes: Assessing and documenting patient condition, including vital signs, medical history, and treatment plans Administering medications and therapies as prescribed by our medical team Developing and implementing individualized care plans to meet each patient's unique needs Collaborating with our interdisciplinary care team to ensure seamless coordination of care Educating patients and their families on their condition, treatment options, and disease management Maintaining accurate and timely records of patient care and treatment About Our Facility: The Avenue at Strongsville is a state-of-the-art rehabilitation and skilled nursing care facility that offers a distinctive experience not commonly found in traditional care settings.
