Remote Care Manager - LTSS & Home Health (Travel)

SPECTRAFORCE

SPECTRAFOR is looking for a dedicated Long-term Service Care Manager in New York to manage a caseload of members with long-term care needs. You will conduct comprehensive assessments and ensure seamless, person-centered care through collaboration with caregivers and health providers. This role involves significant travel for member visits, requiring a Bachelor's degree and relevant experience in Care Management.
