MIH Paramedic — In-Home Post-Hospital Care & Follow-Up

Christian Hospital and Northwest HealthCare

Christian Hospital and Northwest HealthCare is seeking a Mobile Integrated Health (MIH) paramedic to provide post-hospital follow-up care, conduct home physical exams, and collaborate with physicians to ensure continuity of care. Responsibilities include monitoring medications, dressing changes, vital signs, and connecting patients to community resources. Preferred qualifications include a Class (E) license, 2 years of paramedic experience, and Illinois paramedic license with IDPH MIH program #J-18808-Ljbffr Christian Hospital and Northwest HealthCare
