Responsibilities

• Administer prescribed medications and treatments per order, including routine wound care and injections as directed
• Monitor resident conditions, perform ongoing assessments, document vitals, and communicate changes to the healthcare team
• Assist with activities of daily living and contribute to individual care plans under supervision
• Collaborate with registered nurses and physicians to coordinate patient care and respond to acute changes
• Ensure safety, maintain infection control practices, and perform timely, accurate charting and documentation
• Report deviations from expected status promptly and participate in shift handoffs and care conferences

Requirements

• Active LPN license or eligibility to practice as an LPN per applicable regulations
• Ability to work local, Albuquerque-area assignment and 3x12 night shifts (18:00–06:30)
• Experience in long-term care or skilled nursing settings preferred
• Strong assessment, documentation, and communication skills
• Capacity to work in a team-based, patient-centered environment
