Pediatric Collaborative Care Coordinator

SBH Health System is seeking a compassionate and organized Pediatric Collaborative Care Coordinator to support youth ages 1221 across our three pediatric ambulatory care sites. This role ensures timely access to behavioral health services, smooth care transitions, and strong collaboration with Pediatricians, Social Workers, and Behavioral Health providers.

Responsibilities

• Coordinate mental health referrals, follow-up, and care transitions across all pediatric sites
• Partner closely with Pediatricians and Social Workers to support integrated behavioral health care
• Provide youth-centered education on mental health conditions, coping skills, and treatment plans
• Assist with mental health screenings and ensure timely follow-up for elevated scores
• Support crisis response, safety planning, and linkage to urgent services
• Connect families to community mental health, school-based, and social service resources
• Maintain accurate documentation and track high-risk youth for follow-up
• Participate in interdisciplinary case conferences and quality improvement initiatives

Qualifications:

1. Education Required - HS Diploma or Equivalence
2. License/Certification/Registration(s) Required - Graduate from an Approved Medical Assistant Program

• Previous experience in a medical setting or 2 years relevant work experience preferred
• Excellent communication/interpersonal skills
• Bilingual preferred
• Knowledge of computer technology preferred. Successful completion of St. Barnabas Hospital orientation.
