MLR is seeking a dedicated and skilled Home Health Physical Therapist to join our team. In this role, you will provide in-home therapy services to help patients improve mobility, strength, and overall physical function. - Patient Evaluation : Assess patients' physical capabilities and functional limitations in their home environment, and create individualized treatment plans to improve mobility, strength, and overall function.
- Treatment Planning & Execution : Implement therapy programs that focus on improving movement, balance, and independence with daily activities.
- Patient Education : Educate patients and caregivers on exercises, home modifications, and strategies to promote mobility and safety.
- Collaboration : Work closely with physicians, occupational therapists, nurses, and other healthcare professionals to ensure coordinated, holistic patient care.
- Documentation : Maintain accurate, up-to-date records of patient progress, therapy sessions, and outcomes in compliance with home health regulations.

- Education : Graduate of an accredited Physical Therapy program.
- Certification : Current state licensure as a Physical Therapist (PT).
- Experience : Previous experience in home health or outpatient therapy preferred, but new graduates with strong clinical skills are encouraged to apply.
- Skills : Excellent interpersonal and communication skills, strong time management, and the ability to work independently while delivering high-quality care in patients' homes.
