The role involves patient assessments, developing and implementing individualized care plans, and coordinating with physicians and the care team. The therapist delivers or directs most physical therapy to support patients throughout home health and the continuum of care. What You Will Do Provide evaluations and treatment interventions within scope of practice and professional ethics. Assess and evaluate therapeutic/rehabilitative/functional status; participate in patient placement and plan development. Deliver purposeful, skilled interaction care using procedures that produce functional improvements. Observe and report patient reactions and changes in condition. Communicate payor requirements to physicians, treatment team, and families in a timely manner. Prioritize caseload, determine staffing needs, and coordinate assignments/handoffs. Qualifications Graduate of an AOTA‑accredited school with a bachelor, master, or doctoral degree in Occupational Therapy. Current registration with the National Board for Certification in Occupational Therapy and active state OT certification in Colorado. Valid Basic Life Support (BLS) certification for HealthCare Providers. Completed at least one clinical rotation or work experience in acute care, long‑term acute care, skilled nursing, or acute rehabilitation. Occupational Therapist Registered (OTR) status. Bachelor’s degree is acceptable. Other Information No travel required. No previous experience needed.
