Job Summary Signature Rehab is seeking a qualified Physical Therapist to provide rehabilitative care to residents and patients under an integrated care model. $8,000 Sign‑On Bonus available. This role works closely with interdisciplinary clinicians to promote full recovery and timely return to daily activities. Responsibilities Work as a rehabilitative therapist under the integrated care model, collaborating with other clinical caregivers to ensure residents and patients achieve full recovery and timely return to desired activities. Assess and set patient recovery goals and develop individualized therapeutic plans. Educate patients and families on exercises, mobility, and preventive strategies. Maintain accurate and timely documentation of progress and interventions. Qualifications Graduate of an accredited physical therapy program, culminating in a minimum of a Bachelor’s of Science Degree. Valid Physical Therapy license in the state where services are rendered.
