RN Transition Out of Hospital Setting | Home Health | Pittsburgh Area
If you're an RN working in Med Surg, Telemetry, or ER and feeling burned out by long shifts, high patient loads, and constant pressurethis could be a strong next step.
We're working with a growing home health team in the Pittsburgh area that is open to training strong clinical nurses into home-based care, with a structured onboarding program and supportive transition.

The role
- Provide patient care in a home health setting
- Manage patient care plans and documentation
- Travel locally between patient visits
- Work independently while supported by a clinical team
Schedule
- Full-time, MondayFriday
- Flexible daily scheduling (no 12-hour shifts)
- Minimal weekend requirements
Compensation
- Pay-per-visit structure
- $10K sign-on bonus
- Full benefits package
What they're looking for
- Active RN license (PA)
- 1+ year RN experience
- Strong clinical background required (Med Surge, Telemetry, or ER)
- Comfortable working independently and managing your day
- Okay to local travel
Certifications:
- AHA BLS (required)
- ACLS preferred
