Nurse Navigator

The Nurse Navigator (NN) will work collaboratively with the orthopedic Foot& Ankle and Upper Extremity faculty experts and their interdisciplinary teams focusing on the nursing perspective. The NN coordinates the care for the patient prior to the patient's first consult visit at OHSU through the end of the journey for surgery and recovery. This position will work identify patient barriers, coordinate smooth transitions of care, follow the pre and post-operative journey and be responsible for complex patient tracking. This will allow the team to create a plan of care at the patient's first visit and, through the treatment of their orthopedic condition.

Function/Duties of Position

A NN OHSU is a professional RN with orthopedic-specific clinical knowledge who offers individual assistance to patients, families, and caregivers to help overcome healthcare system barriers. The NN guides patients and their families/caregivers through their joint replacement experience, implementing an integrated process of care to facilitate patients' intervention, and recovery. The NN interfaces with research staff and works to identify patients who may be suitable candidates for clinical trials.

Patient Populations: Serving patients aged 16 and above, primarily from all over the metro area along with the entire state of Oregon.

1. Conducts a patient assessment, using the nursing process, to identify patient's understanding of surgery, barriers to care, social determinants of health, preferred learning style, family support, and any current symptoms. This position plays a key role on the interdisciplinary team as the single point of contact to coordinate care in the pre, intra and post-operative care as well as physical therapy leading up to and following a joint replacement performed at OHSU and partner hospitals.

2. The NN tracks data on each referral in order to capture information on the population served and to follow metrics for volume, access, referral sources, work up requirements, subsequent needs after consult.

3. The NN may meets with patients in clinic or on the inpatient unit as determined by the specialty team.

4. Provides pre- and post-op education, educational resources and support to patients and their families/caregivers. The NN acts as a resource to all members of the health care team related to provision of care, enhancing assessment skills of others, establishment of appropriate care standards, and determination of realistic goals for optimal patient outcome

5. The NN ensures when hospitalization is required; patients and their family members/caregivers understand prior to admission, what they can expect during the pre-hospitalization, hospital stay and post discharge period. The NN works with the care coordination team and inpatient team on the admission and discharge plans. The NN will work with the orthopedic total joint APPs on triage of patient specific issues and concerns pre and post-surgery.

6. Collaborate with the Quality team to identify and track key performance measures, including but not limited to: Patient satisfaction, physician satisfaction, LOS, variable cost/case, discharge disposition, and orthopedic specific outcome tools as necessary.

Required Qualifications

• Requires three years of nursing experience
• Minimum two years recent orthopedic operating room or orthopedic direct clinic patient care nursing experience required.
• BSN Graduates: Baccalaureate Degree in Nursing from a program accredited by Commission of Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN) or Commission for Nursing Education Accreditation (CNEA) 30 days before start date.
• ADN Graduates: Associate Degree in Nursing from an accredited program 30 days before the start date.
• Associate degree Nurses required to enroll in BSN program within 3 years of hire and complete within 5 years of hire
• Current, unencumbered Oregon State Registered Nurse License
• BLS from AHA required. New hires will be enrolled and required to complete during orientation.

• External candidates: OHSU & Nursing New Employee Orientation (NEO) for about a week.
• Either Transition to Practice (TTP) Program Specialty Fellowship/Fellowship Learning Pathway or Orientation experience for 3 days-26 weeks depending on the care area. An employment service agreement may apply.
