Care Coordinator

Job Summary


Our client is seeking a dedicated Care Coordinator who will be responsible for collaborating with an interdisciplinary team to ensure comprehensive care coordination. The primary goal is to maintain accurate documentation, build strong member relationships, and continually monitor care plans to meet the healthcare needs of the members.


Key Responsibilities

• Collaborate with an interdisciplinary team to identify each member's care needs.
• Coordinate referrals to external providers and services.
• Maintain accurate documentation for member referrals.
• Build and nurture strong relationships with members and families.
• Monitor care plans to ensure progression on care goals.
• Adhere to HIPAA standards and confidentiality guidelines.
• Facilitate clear communication between care teams and clinicians.
• Assist in problem-solving issues related to healthcare and social barriers.
• Act as a primary system navigator for members and families.
• Perform additional duties as assigned.

Qualifications

• H.S. Diploma or equivalent required; degree or coursework in healthcare preferred.
• Minimum of 2 years of experience in referral management or healthcare coordination.
• Medical Assistant (MA) or medical administrative experience is highly helpful.
• Experience with Electronic Medical Records (EMR) systems, such as eCW.

Skills

• Deep understanding of the healthcare ecosystem and insurance authorization.
• Passion for helping senior members with care plans.
• Proficiency in medical terminology, CPT, and ICD-10 codes.
• Solid understanding of Medicare criteria for medical equipment.
• Exceptional communication and rapport-building skills.
• Proficient computer skills, including Microsoft Office suite.
