Overview

Seeking a compassionate and experienced Long Term Care (LTC) RN to provide skilled nursing care to residents in a long-term care setting. The ideal candidate will be dedicated to delivering high-quality, resident-centered care while collaborating with an interdisciplinary team.

Responsibilities

• Assess, plan, implement, and evaluate resident care plans
• Administer medications, treatments, and nursing interventions
• Monitor resident conditions and report changes as appropriate
• Coordinate care with providers, families, and healthcare team members
• Maintain accurate and timely documentation
• Supervise and support LPNs and CNAs as needed

Requirements

• Active RN License
• Minimum 1 year of LTC, Skilled Nursing, Rehab, or Acute Care experience
• Strong assessment, communication, and organizational skills
• Ability to manage multiple residents in a fast-paced environment
