The Home Health Physical Therapist provides specialized physical therapy services to patients in their homes, focusing on improving mobility, promoting rehabilitation, and enhancing patients' quality of life. This role is an integral part of a multidisciplinary team, requiring collaboration and patient-centered care with minimal travel within the service area. Responsibilities

• Conduct patient assessments to determine physical therapy needs
• Develop individualized treatment plans tailored to patient goals
• Implement therapeutic exercises to restore function and strength
• Provide mobility training to enhance patient independence
• Document patient progress and update care plans accordingly
• Educate patients and families on recovery techniques and safety
• Coordinate care with multidisciplinary team members
• Evaluate patient safety and recommend necessary equipment
• Perform follow-up visits to monitor and adjust treatments

 

Preferred Qualifications

• Minimum 1 year of experience in physical therapy
• Current state physical therapy license
• Strong skills in patient assessment and rehabilitation techniques
• Expertise in mobility training and therapeutic exercises
• Excellent communication and time management abilities
• Demonstrated empathy and patient-centered approach
• Proficient documentation and record-keeping
