At PCGH, your day might begin reviewing physician orders, continue with visits to several residential homes, include teaching Direct Support Professionals, and end by helping solve a clinical concern before it becomes a bigger issue. What your days will look like

No two days are exactly alike, but your responsibilities will include:

• Visiting residential programs to provide nursing oversight and assess health needs
• Reviewing physician orders, Medication Administration Records (MARs), medication storage, and other clinical documentation
• Completing medication reviews and maintaining required nursing records
• Coordinating with physicians, pharmacies, hospitals, therapists, and other healthcare providers
• Providing clinical guidance and support to Direct Support Professionals and program leadership
• Assisting with medication administration training and competency reviews
• Identifying health concerns early and recommending appropriate interventions
• Participating in interdisciplinary planning to help each person achieve the highest possible level of health and independence
• Providing limited on-call nursing consultation for clinical questions

The schedule

This is a full-time position . Most work is performed Monday through Friday during the day or early evenings. We offer reasonable flexibility in scheduling whenever operational needs allow, while recognizing that certain meetings, trainings, home visits, and other responsibilities have established times that require attendance. Occasional evening or weekend hours may be required to meet clinical needs, respond to urgent situations, or support the people we serve.

The nurse we're hoping to find

You're a Registered Nurse who enjoys balancing clinical responsibilities with education, collaboration, and problem-solving. You're comfortable working independently, but you also appreciate being part of a team that communicates well and supports one another.

Required

• Active North Carolina Registered Nurse license
• Strong organizational and documentation skills
• Excellent communication and professional judgment
• Valid driver's license with legal eligibility to drive in North Carolina

Preferred

• Experience providing home visits or community-based nursing services
• Experience supporting adults with intellectual/developmental disabilities and/or traumatic brain injury
• Experience coordinating care across multiple providers and settings
