We are currently searching for a highly qualified, resident‑focused Social Services Designee to join our friendly, caring and supportive team! Responsibilities As a Social Services Designee you will assist in preparing our residents for their future. Your work will ensure our residents receive the high standard of care they have grown to expect at our facility by ensuring our residents social and emotional needs are met during their stay, coordinating resident discharges and participating in resident care conferences. Two (2) years of experience as a Social Services Designee in a Skilled Nursing Facility is preferred. Bachelor’s degree in Social work or similar field. Five (5) years of related experience is preferred.
