Job Overview

The Primary Care Physician serves as the leading physician within a multidisciplinary Patient Aligned Care Team (PACT) or may act as a float or gap provider. This role involves delivering comprehensive, collaborative acute and ongoing care to a designated group of Veterans, as well as to other patients when necessary.

A relocation or recruitment incentive may be offered to exceptionally qualified candidates. Additionally, this position qualifies for the Education Debt Reduction Program (EDRP).

Key Responsibilities
  • Provide continuous care for one or more assigned panels of Veterans.

  • Utilize available tools, including registries, to effectively identify and intervene with individual patients and groups.

  • Ensure that patients within the assigned panel receive appropriate evaluations and access to care.

  • Operate within the full scope of clinical privileges, credentials, and practice guidelines as approved by VHA or local facility documentation.

  • Engage in team performance improvement initiatives to enhance efficiency and patient care delivery.

  • Implement management processes for primary care operations as needed.

  • Facilitate safe transitions for patients between the PACT site and other healthcare environments through effective communication.

  • Deliver health education and coaching on wellness, disease prevention, chronic care management, and self-management skills to patients and their support persons, based on the PACT member's expertise.

  • Encourage patient engagement in their healthcare and involve personal support persons in discussions about VA care.

  • Communicate respectfully and effectively with all team members, including patients and their support persons, to share significant clinical information.

  • Provide healthcare services in accordance with the physician's licensure and clinical privileges.

  • Ensure that the patient's care plan includes medically necessary recommendations.

  • Offer clinically appropriate healthcare services to assigned patients and arrange for care as needed.

  • Guarantee same-day access for both face-to-face and telephone visits during regular clinic hours.

  • Collaborate with PACT staff to create personalized health plans that address care management and coordination based on patient needs.

  • Lead the application of the PACT model and clinical practice guidelines to enhance patient care outcomes and foster high-performing teams.

  • Complete Women's Health Training certification and provide care specific to Women's Health.

  • Deliver gender-specific care in line with VHA directives for Women Veterans, including routine screenings and management of menopause and osteoporosis.

  • May participate in the education and supervision of medical students and residents, ensuring compliance with GME requirements while promoting a supportive learning environment.

  • Respond promptly to patient-related communications from other clinicians, regardless of the communication method, to facilitate coordinated care.

  • Ensure that patients are informed of normal diagnostic results within 14 days and abnormal results requiring action within a clinically appropriate timeframe, not exceeding 7 days.

  • Perform additional duties as assigned by the Chief Medical Officer.
